Support services for people under 65

Our Home and Community Care Program for Younger People (HACCPYP) provides basic support services for eligible people aged under 65 years, or Aboriginal and Torres Strait Islander people aged under 50 years.

The program is designed for people who need support to manage activities of daily living and maintain independence due to chronic illness, disability or mental health challenges, but who are ineligible for the National Disability Insurance Scheme (NDIS).

We have a range of programs that offer opportunities for social interaction, participation in activities of interest, and connection with the community.

Accessing the program

HACCPYP at City of Whittlesea is currently experiencing exceptionally heavy demand. We are not currently accepting new referrals or operating a waitlist for home care, garden maintenance, transport or community access services. We expect referrals will remain closed until July 2026.

We have limited capacity to accept referrals for clients who require personal care, delivered meals, or the installation of grab rails for safety. We are prioritising clients who are most in need of this assistance (for example, people who live alone with limited support).

We have the capacity to accept referrals for social support programs.

Although we are unable to provide the full suite of HACCPYP services at this time, our team is happy to discuss your situation and explore if there are alternative support options in the community. Contact us on 03 9217 2170 (press option 5) and ask to speak to the HACCPYP team.

Where we have the capacity to provide services, a HACCPYP officer will arrange a face-to-face appointment to complete a holistic assessment to understand each individual’s needs. The officer will work with you to develop meaningful goals that allow you to live independently in your home and actively participate in your community. 

Paying and managing your services

  • Invoices are sent monthly and can be sent via email or post
  • Services that occur on public holidays will be automatically suspended
  • Clients will be provided with an invoice at the end of each month
  • Non-payment of fees may result in suspension or cancellation of services
  • Fees will be reviewed in July every year
  • All services have a 1-hour minimum service time
  • The full-service fee is charged where less than 24 hours notice of booking cancellation is provided
  • If you need to go to hospital or are taking a holiday, let us know and we can temporarily suspend your services for up to a six-week period.
